RELEASE OF LIABILITY AND CONSENT FOR MINOR PARTICIPANTS

1. The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN AS ARISING FROM THE NEGLIGENCE OF THE RELEASES of others, and assume full responsibility for my child’s participation; and,
3. I do hereby agree and consent to my child’s participation in PACIFIC COAST HOOPS YOUTH BASKETBALL LEAGUE during the current season, and also assume all risks and hazards which are incidental to the conduct of the activities. I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, ABSOLVE, INDEMNIFY AND HOLD HARMLESS PACIFIC COAST HOOPS YOUTH BASKETBALL LEAGUE, its officers, directors, employees, agents and any of them, their sponsors, organizers, and supervisors of any and all liability or damage, injury, or expense of any kind arising out of or connected with, my child’s participation in PACIFIC COAST HOOPS YOUTH BASKETBALL LEAGUE, a youth basketball league. I do also release and HOLD HARMLESS any and all owners and lessors of premises used to conduct the event.
4. I warrant that my child is in good health, has no condition or defect which would interfere with his/her participation, or would be in any way affected by such participation. I understand that participation in competitive athletics may result in serious injury. I also understand that it is impossible to totally eliminate such occurrences from competitive sports. I/we understand that we can reduce the risk of serious injury by obeying safety rules and complying with the program’s customary terms and conditions for participation.
5. I am hereby informed that all registered participants are covered by an insurance policy in case of accident or medical emergency while participating in an activity sponsored by PACIFIC COAST HOOPS YOUTH BASKETBALL LEAGUE. I further understand that my own personal insurance plan will be used as primary. If I do not have a personal plan, the above insurance will take effect immediately.

I HAVE READ THIS RELEASE OF LIABILITY, CONSENT AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND BY SIGNING ON BEHALF OF MY CHILD, I ASSUME ALL THE RISKS ASSOCIATED WITH PARTICIPATING IN ANY WAY IN THE PACIFIC COAST HOOPS YOUTH BASKETBALL LEAGUE PROGRAM. I SIGN THIS FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
